Post Acute Care Facilities

Our physicians follow patients from Acute Care Hospitals to LTACH for continuity of care and to make sure the patients get quality medical care in these facilities with ultimate goal of sending the patients home safely.

Some patients with multiple medical problems will need higher level of rehabilitation. Our physician team will follow these patients at the Acute Rehabilitation Institute and manage their medical problems to remain medically stable to participate in the rehabilitation program.

Some of the patients require long term nursing and therapy needs due to multiple medical problems. We continue to follow these patients at the nursing facilities to coordinate care and to ensure safe discharge from these facilities to home.
